Diagnostic value of platelets, cytokeratin-19 fragments in patients with primary lung cancer

Abstract

Objective: To determine the concentration of platelets and CYFRA 21-1 in patients with primary lung cancer at the time of diagnosis.

Method: This prospective cross-sectional study was conducted at the time of initial diagnosis on 140 patients with lung cancer, compared to 40 patients with pneumonia. The study measured levels of CYFRA 21-1 and peripheral blood platelets in patients at the Oncology Department of 71 Central Hospital from July 2018 to December 2022.

Results: The mean platelet count in the lung cancer group was significantly lower than in the pneumonia group, at 299.80 ± 113.61 G/L versus 365.51 ± 116.43 G/L (p < 0.05). Conversely, CYFRA 21-1 levels were higher in the lung cancer group compared to the pneumonia group, with mean values of 16.62 ± 10.10 ng/ml and 2.43 ± 2.44 ng/ml, respectively (p < 0.01). The mean platelet count was higher in the small cell lung cancer group compared to the non-small cell lung cancer group, at 377.85 ± 108.78 G/L versus 245.00 ± 124.19 G/L (p < 0.01). Meanwhile, CYFRA 21-1 levels were lower in the small cell lung cancer group than in the non-small cell lung cancer group, with means of 11.74 ± 4.62 ng/ml versus 15.23 ± 1.42 ng/ml (p < 0.01). In terms of tumor size, the mean platelet count in tumors ≤ 7 cm was lower than in those > 7 cm, with values of 304.45 ± 118.15 G/L and 432.04 ± 68.58 G/L, respectively (p < 0.01). CYFRA 21-1 levels in tumors ≤ 7 cm were also lower than in those > 7 cm, with mean values of 21.85 ± 9.00 ng/ml versus 10.93 ± 7.97 ng/ml (p < 0.01).

Conclusion: This study indicates significant differences in mean values of platelets and CYFRA 21-1 between groups. Elevated platelet counts and CYFRA 21-1 levels were more commonly observed in patients with advanced-stage lung cancer.
