Abstract
Background: Proximal hypospadias with severity chordee presents a technical challenge. Many techniques are used to repair proximal hypospadias, but 2 – stage proximal hypospadias is prefer application because of: simple technique, few complications an aesthetics. To describe our experience with 2 - stage proximal hypospadias repair and report outcomes. To look at patient and procedure characteristics associated with surgical complications.
Methods: Case series of children having proximal hypospadias were repaired in 2 stages at Children’s Hospital No.2 from January 2019 to Jun 2020.
Results: A total of 34 cases are detected. The median age at first stage surgery was 3.7 ± 2.1 ages. The median time between surgeries was 8.4 ± 1.7 months. At the first stage, the maneuver used for straightening included degloving in 17/34 (50.0%), dorsal plication in 16/34 (47.1%) and dermal graft in 1/34 (2.9%). At the second stage, all patients had straight penis. Urethroplasty by Duplay procedure in 33/34 (97.1%), tubularized incised plate in 1/34 (2.9%). Neourethra was covered by penile dartos flap in 29 (85.3%), penile and scrotal dartos flap in 5 (14.7%). The mean length of follow - up was 4.4 months (1.0 - 8.0). Surgical complications were reported in 14/34 (41.2%) patients; including: fistula in 6 (17.6%), dehiscence in 6 (17.6%), meatal stenosis in 2 (5.9%), residual chordee in 1 (2.9%), penile torsion in 5 (14.7%). Rate of redo surgery was 20.6%.
Conclusions: Although final complication rate is rather high (41.2%) but the reoperation rate is acceptable (20.6%)
