Abstract
Background: Cisplatin is a widely used potent chemotherapy agent in the treatments of various malignant tumors in children. However, Cisplatin is reported to have several side effects. A case treated with Cisplatin and having acute kidney injury and posterior reversible encephalopathy syndrome was observed.
Case report: A case of a 6 - years - old boy was diagnosed retroperitoneal neuroblastoma stage 4and was treated with Rapid COJEC regimen. On day 48, after 2 cycles of chemotherapy using Cisplatin, our patient developed hypertension and acute kidney injury. After 3 cycles of Cisplatin, on day 60, he had headache, focal seizure, and transient loss of consciousness. Brain MRI scan revealed edema in the cortical gray and subcortical white matter of the bilateral occipital, parietal and temporal lobes which were hyperintense only on FLAIR images, consistent with posterior reversible encephalopathy syndrome. After 40 days of treatment with fluid and electrolyte balance, renal diet, antihypertensive and anticonvulsant drugs, our patient’s condition has improved. He hasn’t had anyfurther seizure episodes. Blood pressure has been controlled and kidney function has returned to almost normal. His brain MRI showed complete resolution.
Conclusion: Pediatrician oncologists need to promptly detectany side effects while using Cisplatin to manage and switch to other drugs
