Đặt vấn đề: Bạch cầu cấp dòng lympho là một bệnh ác tính hay gặp nhất ở trẻ em, sốt giảm bạch cầu hạt là một trong những biến chứng nguy hiểm trong điều trị bệnh bạch cầu cấp dòng lympho. Mục tiêu: Đánh giá kết quả điều trị sốt giảm bạch cầu hạt ở bệnh nhân bạch cầu cấp dòng lympho. Phương pháp và đối tượng nghiên cứu: Phương pháp nghiên cứu mô tả cắt ngang trên 90 lượt bệnh nhân từ 1 tháng đến 15 tuổi được chẩn đoán bạch cầu cấp dòng lympho có sốt giảm bạch cầu hạt sau khi bắt đầu điều trị hóa trị liệu, từ tháng 02/2013 đến tháng 06/2014. Kết quả: Không có trường hợp nào tử vong. Nhóm không có điều chỉnh kháng sinh (KS) sau liệu pháp KS ban đầu chiếm 64,4% so với 35,6% của nhóm có điều chỉnh KS. Ở nhóm không điều chỉnh kháng sinh, KS được dùng ban đầu theo kinh nghiệm là Ceftazidime chiếm 65,5% và kế đến là phối hợp Amikacin + Ceftazidime chiếm 24,1%. Ở nhóm có điều chỉnh kháng sinh, phối hợp KS Meropenem + Amikacin chiếm 59,4%, kế đó là Meropenem + Vancomycin 18,8%, và Meronem đơn độc là 12,5%. Thời gian kéo dài của sốt ở nhóm có điều chỉnh KS với trung vị 4 ngày so với 2 ngày ở nhóm không điều chỉnh KS (p<0,01). Thời gian trung vị sử dụng kháng sinh ở nhóm không điều chỉnh KS là 7 ngày so với 12 ngày ở nhóm có điều chỉnh KS (p<0,01). Thời gian trung vị để bạch cầu hạt đạt đến mức 500/mm³ là 8 ngày ở nhóm có điều chỉnh KS và 6,5 ngày ở nhóm không điều chỉnh KS (p>0,05). Thời gian trung vị để bạch cầu hạt đạt đến mức 1000/mm² là 12 ngày ở nhóm có điều chỉnh KS và 10 ngày ở nhóm không điều chỉnh KS (p>0,05). Kết luận: Chẩn đoán sớm và điều trị tích cực theo đúng phác đồ sẽ cải thiện đáng kể kết quả điều trị sốt giảm bạch cầu hạt ở bệnh nhân bạch cầu cấp dòng lympho.
