Results of treatment of neutropenia fever in children with acute lymphoblastic leukemia

Abstract

Backgrounds: Acute lymphoblastic leukemia is the most common malignant disease in children, in which neutropenia fever is one of the most common and serious complications in the treatment of acute lymphoblastic leukemia. Objective: To evaluate the results of treatment of neutropenia fever in children with acute lymphoblastic leukemia. Method and Patients: A cross sectional descriptive study on 90 episodes of neutropenia fever after starting chemotherapy from 02/2013 to 06/2014 in 49 children aged from 1 month to 15 year old diagnosed with acute lymphoblastic leukemia at Pediatrics center of Hue Central Hospital. Results: No case of death. Group without initial empirical antibiotherapy modification accounts for 64.4% compared. In group without antibiotherapy modification, initial empirical antibiotics including Ceftazidime accounts for 65.5%, followed by the combination of Amikacin + Ceftazidime (24.1%). In group with antibiotherapy modification, a combination of Meropenem + Amikacin accounts for 59.4%, followed by Vancomycin + Meropenem (18.8%), and monotherapy of Meronem (12.5%). The median duration of fever in group with antibiotherapy modification is 4 days compared to 2 days in group without antibiotherapy modification (p <0.01). The median duration of antibiotics use in group without antibiotherapy modification is 7 days compared to 12 days in group with antibiotherapy modification (p <0.01). The median duration, in which the absolute neutrophile count reachs to level of 500/mm³, is 8 days in group with antibiotherapy modification and is 6,5 days in group without antibiotherapy modification (p>0.05). The median duration, in which the absolute neutrophile count reachs to level of 1000/mm³, is 12 days in group with antibiotherapy modification and is 10 days in group without antibiotherapy modification (p>0.05). Conclusion: Early diagnosis and prompt antibiotics treatment in accordance with the protocol will significantly improve the outcomes of neutropenia fever in children with acute lymphoblastic-leukemia.
