Abstract
Background: About 80% preterm newborns have hyperbilirubinemia in first week of life. Approach to initial screening for hyperbilirubinemia in preterm newborns by cord blood is practical, cheap and noninvasive. This study aimsto describe some relations between cord blood albumin, bilirubin concentration and bilirubin/albumin ratio with characteristics and hyperbilirubinemia classification of preterm newborns.
Method: A prospective cohort study was carried out all preterm newborns < 37 weeks, were born at Hospital of University of Medicine and Pharmacy, Hue, Viet Nam
from 4/2018 to 8/2020. Cord blood albumin and bilirubin was collected after birth. Neonates were followed up daily. Recording all information about the medical history
of infants and their mother in the research sheet.
Results: Out of the total 176 neonates enrolled, the ratio of males to females was 1.26/1, the mean gestational age was 34.5 ± 1.4 weeks, the mean birth weight was
2190.0 ± 425.5 grams, 83% of them was appropriate for gestation age. The mean value of cord blood albumin concentration was 3.44 ± 0.35 g/l, cord blood bilirubin
concentration was 1.77 (1.50 - 2.01), bilirubin/albumin ratio was 0.52 (0.42 - 0.60). There was a statistically significant difference between cord blood bilirubin and bilirubin/ albumin ratio with gestational age, birth weight and hyperbilirubinemia classification of preterm newborns (p < 0.05).
Conclusion: In preterm newborns, cord blood bilirubin concentration and bilirubin/ albumin ratio of the pathological hyperbilirubinemia group were higher than the physiological group.
Key words: Neonatal hyperbilirubinemia, preterm, cord blood albumin, cord blood bilirubin, bilirubin/albumin ratio
