Abstract
Background: The term inflammatory bowel disease (IBD) is used to represent 2 distinctive disorders of idiopathic chronic intestinal inflammation: Crohn disease and ulcerative colitis. Their respective etiologies are poorly understood, and both disorders are characterized by unpredictable exacerbations and remissions. The most common time of onset of IBD is during the preadolescent/adolescent era and young adulthood. Children with very early-onset (VEO-IBD) that presents in children < 6 years of age are uncommon (about 1%), especially under 1 year of age (0.2%). Ulcerative colitis, an idiopathic chronic inflammatory disorder, is localized to the colon and spares the upper gastrointestinal tract. Disease usually begins in the rectum and extends proximally for a variable distance. When it is localized to the rectum, the disease is ulcerative proctitis, whereas disease involving the entire colon is pancolitis.
Case report: We are presenting a case of very early-onset ulcerative colitis in a 12 months old boy who presented with watery stools with blood in the stools lasting 4 months, without fever or other unusual symptoms. Diagnosis was confirmed by colonoscopy and biopsy. Treatment was thereafter started with oral corticosteroid and mesalazine. The patient is now on remission and is on regular follow up.
Conclusion: Ulcerative colitis is an idiopathic chronic inflammatory disorder of the colon. Ulcerative colitis should be suspected in children with persistent bloody diarrhea. With early diagnosis and appropriate treatment with immunosuppressive therapy, children can lead a healthy life.
