Abstract
Bối cảnh: Lupus ban đỏ hệ thống (SLE) là một rối loạn tự miễn phức tạp đặc trưng bởi sự tham gia của nhiều hệ thống, bao gồm cả hệ thần kinh. Đây là một biến chứng nghiêm trọng với tỷ lệ tử vong và di chứng cao. Nghiên cứu này nhằm đánh giá các biểu hiện thần kinh tâm thần của bệnh lupus hệ thống ở trẻ em.
Phương pháp: Trong một nghiên cứu loạt ca hồi cứu đã tuyển dụng 17 bệnh nhân từ ngày 1/6/2018 đến ngày 1/6/2020.
Kết quả: Độ tuổi trung bình của bệnh nhân là 10,82 ± 1,74 tuổi. Tỷ lệ nữ/nam là 15:2. Các triệu chứng phổ biến nhất là co giật, đau đầu và rối loạn nhận thức, chiếm 64,70%, 58,82% và 29,40%. Điểm SLEDAI cao và mức bổ thể huyết thanh giảm được tìm thấy ở tất cả các bệnh nhân. Hơn nữa, ANA huyết thanh được tìm thấy dương tính ở 82,40% và mức độ anti-dsDNA được tìm thấy tăng cao ở 94,10% bệnh nhân. 46,67% bệnh nhân có phát hiện bất thường trên MRI đầu.
Kết luận: Biểu hiện thần kinh tâm thần là một trong những biến chứng nghiêm trọng phổ biến nhất của SLE nhi khoa. Trong giai đoạn khởi phát bệnh hoạt động, các bất thường thần kinh chính là co giật, đau đầu và rối loạn nhận thức. Do đó, cần theo dõi chặt chẽ trong giai đoạn chẩn đoán và điều trị SLE để giảm tỷ lệ mắc bệnh và tử vong.
