Abstract
Background: Airway foreign bodies are objects lodged in the airways from the larynx to the segmental bronchi. It is a typical emergency in children, rarely seen in adults, and diagnosis can be delayed in the elderly. Neglected airway foreign bodies are often discovered incidentally or when complications of the disease recur. This report describes the case of an elderly patient with a foreign body in the main bronchus, large size, neglected for more than 15 years, who was diagnosed and successfully intervened endoscopically to remove the foreign body at Hue Central Hospital.
Case report: A 75 - year - old male patient was admitted to the hospital because of a persistent cough. History suspected of choking on Sapote nut while eating (>15 years) with transient infiltrative syndrome, later presenting with recurrent lower respiratory tract infections. The patient was examined at a local medical facility but no foreign body was detected. About 1 month before admission, the patient coughed with little blood, so a CT scan of the lung was performed to detect foreign bodies.
Diagnosis: Chronic bronchitis/Forgotten airway foreign body. The patient underwent bronchoscopy with rigid and flexible tubes to remove all foreign bodies.
Conclusion: The diagnosis of an airway foreign body must be performed by an ENT doctor, and taking a history of foreign body exposure with invasive syndrome is crucial to avoid missed cases. Interventional treatment with rigid and flexible bronchoscopy provides optimal efficiency in removing large bronchial foreign bodies.
