Combined saphenous nerve block and sciatic nerve block under the guidance of ultrasound for lower extremity amputation in high-risk patients

Abstract

The lower extremity amputation is indicated for extensive damage or infection in the legs and feet that is no longer capable of repair. Patients scheduled for lower extremity amputation often have many underlying diseases such as diabetes, limb artery disease, heart failure, kidney failure, sepsis,... Therefore, anesthetic methods such as general anesthesia with endotracheal intubation and spinal anesthesia have high risks during and after surgery. The saphenous nerve block and the sciatic nerve block under ultrasound guidance with a mixture of Bupivacaine 0.25% and Lidocaine 1% has little effect on the patient’s body, has good quality of anesthetic effect for surgery on the leg and foot. Evaluation of the results on 05 patients performed at Huu Nghi Hospital initially showed good quality of anesthesia, effective pain relief after surgery and no complications were recorded. The onset time of sensory block in the legs and feet is 17.6 ± 2.7 minutes; the quality of emotionlessness is good at 80%. VAS scores for both the rest and the movement at 0 hour, 2 hours, 5 hours, and 10 hours after the blockade were smaller 4. The duration time of sensory blockade was 14.4 hours. The method combined saphenous nerve block and sciatic nerve block under ultrasound guidance with a mixture of Bupivacaine 0.25% and Lidocaine 1% for lower extremity amputation in high-risk patients has initially shown feasible, safe, and effective. Future research with a larger sample size is needed to evaluate the effectiveness, advantages, and disadvantages of the method.
