Abstract
Background: Bladder cancer is a common malignancy in the urogenital organs. With the advancement of imaging modalities, bladder cancer is increasingly being diagnosed early and accurately. This study aims to determine the diagnostic value of computed tomography (CT) in determining the spread in bladder cancer (compared with operative results).
Methods: A retrospective study was conducted on 194 bladder cancer patients who underwent operation at Binh Dan Hospital from January 2020 to June 2020. All patients were preoperatively performed an CT scan and then compared with the operative results to determine the diagnostic value of CT
Results: CT had a high value in the diagnosis of extramural invasion (≥T3b) of bladder cancer (sensitivity 97.4%, specificity 89.7%). Value of CT scan in diagnosis of T - stage: High accuracy of 91.2% in diagnosis of stage <T3b. At stage T3b: sensitivity 78.3%, specificity 98.3%, accuracy 95.9%. At stage T4a, CT had a very high value of 90% or more for invasion of organs such as ureters, prostate, and gastrointestinal tract.
Less sensitive in diagnosis of the invasion of uterus and seminal vesicles (sensitivity range from 40 - 50%), but specificity and accuracy were very high above 95%. At stage T4b, CT had a sensitivity, specificity and accuracy of 99 - 100%.
Conclusion: CT has a high diagnostic value in determine the invasion in bladder cancer
