Efficacy of preoperative chemoradiation therapy in rectal cancer at Hue Central Hospital

Abstract

Objective: Evaluating the efficacy of preoperative chemoradiotherapyand relating factors in patients with T3, T4, non - distant metastatic rectal cancer.

Methods: Thirty - seven patients with T3, T4, no metastasis rectal cancer were treated with radiotherapy (45 Gy/25 fractions) and concomitant Capecitabine (825 mg/m2 bid on each day of the radiotherapy period) at Hue Central Hospital between February 2019 and July 2021. All patients were evaluated for response to the treatment using RECIST 1.1 and toxicities according to CTCAE v5.0.

Results: The mean age was 56.6 ± 12.5 years; with male:female ratio of 2.4:1. The most common symptom was hematochezia (86.5%). The majority of tumors were stage 4 (62.5%) and were located in distal rectum (56.8%). There were 70.3% patients with T3 and 59.5% with positive node. The proportion of stage II and III were 40.5% and 59.5%, respectively. Adenocarcinoma accounted mostly for 97.3%. After neoadjuvant treatment, tumor downstaging rate were 48.6% and nodal downstaging rate were 48.6%. Overall clinical response and complete clinical response rates were achieved by 51.3% and 16.2% of the patients, respectively. There was a correlation of overall response rate after treatment with sex, location and stage of tumor. No patient had grade 4 toxicity. Grade 3 toxicity occurred in only one patient (2.7%) with anemia, the remaining were grade 1 and 2 toxicities which took dominate rate.

Conclusion: Preoperative chemoradiotherapy in T3, T4, no metastasis rectal cancer is an effective and safe treatment with an acceptable toxicity profile.
