Value of GRACE score (2.0) and TIMI in patients with non - ST elevation myocardial infarction with coronary angiography

Abstract

Objectives: To evaluate the relationship between coronary artery lesion characteristics and GRACE and TIMI scores in patients with non-ST elevation myocardial infarction.

Methods: A cross-sectional descriptive study was conducted on 92 patients diagnosed with non-ST elevation myocardial infarction treated at the Department of Interventional Cardiology at Hue Central Hospital and the Department of Cardiology at Hue Central International Hospital from January 2019 to July 2020.

Results: Coronary artery lesions were mainly LAD lesions, accounting for 81.5%. Considering the number of coronary arteries damaged, lesions of the two coronary arteries accounted for the highest rate (40.2%). The mean GRACE score in the group with no or mild lesions as measured by the Gensini score was significantly lower than that in the group with moderate-severe lesions (117.40 ± 22.70 vs. 130.18 ± 20.07, p < 0.05), whereas the mean TIMI score in the two groups did not differ (p > 0.05). There was a weak positive correlation between the GRACE score and the Gensini score with r = 0.394 with p < 0.05, whereas there was no correlation between the TIMI score and the Gensini score (p > 0.05). The GRACE score was valuable in predicting moderate and severe obstructive coronary artery disease, but the TIMI score was not. The area under the curve (AUC) of GRACE was 0.663 (95% CI: 0.552- 0.773; p < 0.05). The GRACE score at the optimal cut-off point of 125.5 had a sensitivity of 55% and a specificity of 30.8%.

Conclusion: There is a correlation between GRACE score and the number of coronary arteries affected, there is a correlation between GRACE score and Gensini score as well as the ability to predict the presence of moderate-severe obstructive coronary artery disease, which was not seen in the TIMI score. However, the early use of risk scores in acute coronary syndromes will be valuable for the assessment and management of patients with coronary artery disease.
