Abstract
Backgroud: Ejection fraction is an important prognostic indicator in heart valve surgery. Reduced and severely reduced ejection fraction are factors that predict mortality and severe complications after surgery.
Methods: From March 2020 to March 2023, 78 patients with heart valve disease with ejection fraction ≤ 40% underwent mitral and aortic valve replacement surgery at Central Hospital. Hue district.
Results: Mean age 41.01 ± 12.04; NYHA III accounts for 75.64%, NYHA IV 14.10%. Preoperative atrial fibrillation 64.40%. Large cardiac shadow on X-ray 79.20%. Average ejection fraction was 37.26 ± 2.74%. The group with ejection fraction ≤ 35% accounted for 24.3%. Systolic left ventricular diameter 50.55±8.59 mm. Systolic pulmonary artery pressure 48.97 ± 16.85 mmHg. In the postoperative period, 2 or more types of inotrope were used, accounting for 55.13%. Support for Intra - aortic ballon pump in 16.66%, indicated intra-operating account in 2.56%. Extracorporeal membrane oxygenation body (ECMO) 1.28% but no response. Early results: 2.56% mortality. After 6 months there was no death.
Conclusion: Using of mechanical supports for ventricular supporting intra and post - operative helped minimize mortality and improve clinical outcome after surgery
