The effect of post - operative analgesia after knee arthroscopy by ultrasound - guided adductor canal block

Abstract

Background: Effective analgesia after knee arthroscopy plays an important role because it facilitates early ambulation and reduces postoperative complications. This study was designed to evaluate the efficacy of adductor canal block in pain management after knee arthroscopy.

Methods: In this prospective study, 70 patients scheduled for knee arthroscopy under spinal anesthesia were randomised to receive ultrasound - guided adductor canal block with 20 mL levobupivacaine 0.25% (group OCK) or not use (group Sample). The block procedures were performed at the end of the surgery when patients were transferred to the recovery area. All patients were given acetaminophen 1 g IV and 75 mg IM every 12 hours within the first 24 hours after surgery. We recorded visual analogue scales (VAS) for pain at rest and on movement, heart rate, blood pressure, respiratory rate, SpO2 (at 0, 1, 3, 6, 9, 12, 18 and 24 hours postoperative), total dose of morphine within 24 hours, patient’s satisfaction and side effects in both groups

Results: VAS in the group OCK was significantly lower than that in the group Sample, including VAS for pain at rest at 1 - 12h, and VAS for pain on movement at 1 - 12 hours after knee arthroscopy. There was no difference in morphine
consumption. There was significantly difference between both the groups regarding patient’s satisfaction (77,14% in group OCK and 65,71% in group Sample), and no difference incidence of postoperative nausea and/or vomiting and incidence of pruritus

Conclusion: The adductor canal block provided analgesia efficacy after knee arthroscopy. Therefore, the adductor canal block is feasible and effective as a part of multimodal analgesia after knee arthroscopy.
