Evaluation of the effectiveness of cold atmospheric plasma as an adjunctive treatment for perineal wounds.

Abstract

Objective: To evaluate the effectiveness, safety, and maternal satisfaction associated with the use of PlasmaMED cold atmospheric plasma as an adjunctive treatment for perineal wounds after vaginal delivery.

Methods: A controlled interventional study was conducted among 472 women who underwent vaginal delivery with sutured perineal wounds at the Postpartum Department, Center for Obstetrics and Gynecology, Hue Central Hospital, from April 2026 to July 2026. Participants were allocated in a 1:1 ratio to two groups: the PlasmaMED group (n=236), which received routine perineal wound care combined with once-daily cold plasma treatment, and the control group (n=236), which received routine wound care alone. Wound status was assessed before intervention, after the first treatment session, and after completion of the entire treatment course. Adverse events and maternal satisfaction were also recorded.

Results: After the first intervention, the PlasmaMED group had significantly lower rates of difficulty sitting or walking, pain during movement, swelling, and wound exudate or purulent discharge compared with the control group (p<0.05). After completion of treatment, the PlasmaMED group showed significantly lower rates of wound tightness or burning sensation (5.1% vs. 10.6%; p=0.03), pain during movement (3.4% vs. 8.5%; p=0.03), pain or burning during urination (0.8% vs. 4.7%; p=0.02), swelling (3.8% vs. 13.6%; p<0.001), and wound exudate or purulent discharge (0.4% vs. 5.9%; p=0.001). The mean time to complete wound dryness was significantly shorter in the PlasmaMED group (1.60±0.65 vs. 2.22±0.61 days; p<0.001). Adverse events were uncommon, mainly including local itching or irritation (2.1%) and mild transient increase in pain after treatment (1.7%); no cases of excessive heat sensation or skin burns were recorded. The proportion of women reporting being very satisfied with PlasmaMED ranged from 85.6% to 93.2%.

Conclusion: PlasmaMED cold plasma treatment improved perineal wound-related symptoms, including pain, swelling, and wound exudate or purulent discharge, while significantly shortening the time to complete wound dryness. The intervention demonstrated a favorable safety profile and a high level of maternal satisfaction.
