Abstract

Background: Endoscopic retrograde cholangiopancreatography (ERCP) is the first-line treatment for common bile duct stones. Research indicates that periampullary duodenal diverticulum (PAD) has an impact on the therapeutic efficacy of ERCP for common bile duct stones.

Methods: A retrospective and cross-sectional descriptive study was conducted on 104 patients with CBD stones who underwent ERCP at Hue University of Medicine and Pharmacy Hospital from January 2021 to August 2025. These patients were divided into two groups: those with PAD (54 patients) and those without PAD (50 patients).

Results: The mean age of the PAD group was significantly higher than that of the non-PAD group. In the PAD group, the successful biliary cannulation rate was 90.7%, the successful stone extraction rate was 79.6%, and the complication rate was 9.3%, which included acute pancreatitis (7.4%) and early bleeding (1.9%). Type I PAD was associated with a decreased likelihood of successful biliary cannulation (OR = 0.06, 95% CI: 0.01 – 0.66).

Conclusion: ERCP is a safe and effective procedure for patients with PAD. Type I - PAD is associated with a decreased likelihood of successful biliary cannulation
