Abstract
Background: Video-assisted thoracoscopic segmentectomy guided by indocyanine green (ICG) is a novel technique that allows for radical resection while preserving lung parenchyma in early-stage non-small cell lung cancer (UTPKTBN). This case report presents one of the first successful applications of this method in Vietnam, conducted at K Hospital.
Case report: A 72-year-old female was diagnosed with stage IA lung adenocarcinoma located in segment 10 (S10) of the left lung. She underwent video-assisted thoracoscopic segmentectomy with intraoperative ICG fluorescence guidance. The operation lasted 90 minutes. The intersegmental plane was clearly delineated under near-infrared fluorescence imaging following intravenous ICG injection. The patient experienced no intra- or postoperative complications. The chest tube was removed on postoperative day 2, and the patient was discharged on day 5. Histopathological analysis confirmed adenocarcinoma with negative surgical margins.
Conclusions: ICG-guided VATS segmentectomy is a safe and effective technique for early-stage lung cancer, offering both oncological adequacy and optimal functional lung preservation. It is especially beneficial for elderly patients or those with limited pulmonary reserve.
