Study on the Function of the Adrenal Glands by Synacthen Test in Pediatric Nephrotic Syndrome After Prednisolone Attacks Treatment

Abstract

Background: The prolonged use of prednisone in the treatment of nephrotic syndrome caused secondary adrenal insufficiency due to the hypothalamic - pituitary - adrenal suppression.

Objective: to evaluate the functions of the adrenal gland and the factors involved in adrenal function by Synacthen (tetracosactide) test in pediatric nephrotic syndrome after prednisolone treatment attacks.

Methods: The study was performed on 30 patients with nephrotic syndrome who were treated by prednisolone 2mg/kg/day for at least 4 weeks.

Results: The rate of adrenal insufficiency: was 93.33%. There was no relationship between adrenal function with time of treatment and number of treatment (p>0.05). There was no relationship between blood cortisol levels 0 and 0' (p>0.05). There was a negative correlation between blood cortisol levels 0' and ratio cortisol 30'/cortisol 0' (rs = -0.875 p<0.05,.
