Abstract
Background: Recently, there has been increasing concern regarding the potential for abdominal wall
morbidity following breast reconstruction with pedicled transverse rectus abdominis myocutaneous (TRAM)
flaps.
Objective: The purpose of this study was to analyze consecutive series of pedicled TRAM flaps
performed by a single surgeon and examine the incidence of abdominal morbidity.
Patients and method: Since January 2017 to June 2019, 47 women underwent breast reconstruction
with unilateral (46 case ) and bilateral (1 case ) pedicled TRAM flaps. All abdominal repairs were reinforced
with polypropylene mesh. The charts were reviewed retrospectively for abdominal complications and
associated demographics.
Results: In 47 patients, there was two abdominal bulges (4.26%), no hernia, two partial skin loss
(4.26% ), three seromas (6.38%) and four peritoneal perforation. Three patients had prolonged abdominal
wall discomfort that was treated successfully with physical therapy. No patient developed a mesh infection
or required mesh removal secondary to infection or exposure and no patien complained of persistent
abdominal weakness.
Conclusions: The pedicled TRAM flaps remain a good option for many women seeking breast
reconstruction.
