Oral cryotherapy for prevention of chemotherapy-induced oral mucositis in children with cancer

Abstract

Background: Oral mucositis is a common complication in the chemotherapy treatment of children. The oral cryotherapy during the chemotherapy infusion has been found to reduce the incidence and severity of oral mucositis. This study aims to investigate the prevalence of oral mucositis in pediatric patients undergoing chemotherapy and to compare the incidence and severity of mucositis between two groups, one with and one without the use of oral cryotherapy.

Methods: A controlled prospective study was conducted on 100 pediatric patients diagnosed with cancers requiring chemotherapy at the Pediatric Center, Hue Central Hospital from April 2018 to May 2023.

Results: In 100 diagnosed pediatric patients, different types of cancers were observed with a male - to - female ratio of 1.43:1. The mean age was 7.3 ± 2.1 years. Statistically significant differences were found in the incidence and severity of oral mucositis between the two groups (p < 0.05). The incidence of oral mucositis in the cryotherapy group was 24.0%, compared to 56.0% in the non - cryotherapy group. Among the cryotherapy group, the incidence of mucositis severity I, II, III, and IV were 50.0%, 41.7%, 8.3%, and 0%, respectively. In the non - cryotherapy group, the incidence of mucositis severity I, II, III, and IV were 28.6%, 50.0%, 17.9%, and 3.5%, respectively. The onset of oral mucositis was significantly earlier in the non - cryotherapy group than in the cryotherapy group (5.8 ± 2.3 days vs. 7.2 ± 1.3 days). The recovery time of oral mucosa was faster in the cryotherapy group than in the non - cryotherapy group with statistical significance (4.0 ± 1.5 days vs. 7.5 ± 2.5 days). The cryotherapy group exhibited lower rates of fever and diarrhea associated with oral mucositis compared to the non - cryotherapy one (p < 0.05).

Conclusion: Clear differences found in the incidence and severity of oral mucositis between the group with and without oral mucosal cryotherapy during the chemotherapy process in pediatric cancer patients.
