Abstract
Introduction: Acute heart failure (AHF) is a critical condition associated with significant morbidity and mortality. Peripheral oxygen saturation (SpO2) is a non-invasive parameter that reflects oxygenation status and can provide valuable insights into the hemodynamic and respiratory impact of heart failure. Understanding the variations in SpO2 among AHF patients and its correlation with clinical and functional parameters, such as LVEF and NYHA classification, can aid in optimizing diagnostic and therapeutic strategies. This study investigates the differences in SpO2 between AHF patients and healthy controls and examines the relationship between SpO2 and clinical variables in the context of AHF.
Methods: The study included 161 participants, divided into two groups: 49 patients with AHF and 112 healthy controls without heart failure or lung diseases affecting SpO2. The mean age of AHF patients was 76.53 ± 13.28 years, with females accounting for 63.3%. Heart failure with preserved ejection fraction (HFpEF) was the most prevalent subtype (59.2%), while mildly reduced ejection fraction (HFmrEF) was the least common (18.4%). Peripheral oxygen saturation and arterial oxygen saturation were measured and analyzed alongside demographic and clinical parameters.
Results: A statistically significant difference in mean SpO2 was observed between the AHF group and controls (p<0.05). Within the AHF group, SpO2 showed a relationship with LVEF categories, although this was not statistically significant (p>0.05). SpO2 negatively correlated with heart rate (p=0.04, r=-0.28) and positively correlated with arterial oxygen saturation (SaO2) (p=0.03, r=0.59).
Conclusion: Acute heart failure predominantly affects individuals over 60 years, with a higher prevalence among females. Mean SpO2 differs significantly between AHF patients and healthy controls, reflecting its potential clinical relevance. Within the AHF group, SpO2 is weakly associated with heart rate and moderately correlated with arterial oxygen saturation, suggesting its utility in patient assessment and monitoring. Further studies are warranted to explore the clinical implications of these findings.
