Abstract

Objective: To describe the clinical features of patients with phacomorphic glaucoma and estimate result
of phacoemulsification for intumescent cataract. Participants and methods: Prospective descriptive research with clinical intervention in 30 eyes of 30
patients with secondary glaucoma due to intumescent cataract after phacoemulsification in Hue Central
Hospital - 2nd Branch from January 2019 to June 2019.
Results: 30 patients consited of 12 men (40%) and 18 women (60%). The mean age of patients was
81.3±5.6 years old. Surgeried eye consited of 14 right eyes (46.7%), and 16 left eyes (53.3%). The mean
VA before and after surgery 1 day, 1 week, 1 month and 3 months was in turn 0.03 ± 0.02; 0.08±0.06;
0.44±0.07; 0.63±0.08; 0.65±0.09. The mean IOP at the point of time before and after surgery 1 day, 1 week,
1 month and 3 months was in turn 40.67±6.2mmHg; 23.47±1.9mmHg; 16.8±1.3mmHg; 14.9±1.4mmHg.
100% patients with white intumescent cataract. The mean time of surgery time was 82.0±11.8 seconds.
Difficulties during surgery: 4 cases with pupil synechiae (13.3%), 6 cases with Zinn weakness (20%),
7 cases of anterior chamber instability (23.3%). Complications in surgery: 4 cases with incision burn
(13.3%), 1 case with Zinn tear (3.3%), 1 case with posterior capsular tear (3.3%), 5 cases with CCC failure
(16.7%). Complications after surgery: 12 cases had corneal edema (40%), 1 case had uveitis (3.3%).
Conclusions: Phacomorphic glaucoma causes severe decrease of visual acuity in elderly people.
There were many difficulties during surgery like Zinn weakness, anterior chamber instability. The more hard
lens, the longer the phaco time, the more phaco energy used. This caused corneal edema, incision burn.
Visual acuity significantly improved and IOP was controlled after surgery in all patients.
