Abstract

Objectives: To determine the relationship of some clinical and subclinical factors with acute complicated appendicitis in children.

Methods: A total 120 pediatric patients who diagnosed with acute appendicitis based on clinical characteristics, laboratory tests (white blood cell count) and ultrasound results underwent appendectomy. The operative findings were determined as: uncomplicated appendicitis and complicated appendicitis. Clinical and subclinical factors were compared between the two groups of complicated and uncomplicated appendicitis to find the related factors. Results: Related factors of acute complicated appendicitis in children were: age ≤ 5, taking the drug before admission (antipyretic, analgesic, antibiotic), time from onset to hospital admission ≥ 24 hours, WBC count> 15 × 109 / L, with appendic stool stones. The relationship between sex, geography, appendix location and complicated appendicitis were not significant.

Conclusion: The diagnosis of appendicitis in children requires a combination of clinical symptoms, laboratory tests and abdominal ultrasound, especially in the case of atypical clinical manifestations. Early diagnosis, especially in young children is needed to reduce the rate of complicated appendicitis.
