Abstract

Background: Thyroid cancer is the leading cause of pediatric endocrine cancer. The rate of thyroid nodules and the malignancy in children are differences from those in adults. This study aims to distinguishing benign and malignant thyroid nodules in children on ultrasound, determine the sensitivity and specificity of imaging characteristics ultrasound and the concordance of the TIRADS classification in diagnosis of thyroid cancer in children.

Methods: A cross-sectional study was conducted in all pediatric patients with thyroid nodules. The inclusion criteria were patients who fully described by ultrasound and having surgery with histopathological results at Children's Hospital 1 from January 2021 to the end of August 2024.

Results: The study included 65 pediatric patients aged 8 to 16 years. The following ultrasound features were valuable in differentiating benign and malignant thyroid nodules: solitaire, nodule size, thyroid volume, solid composition, hypoechogenic, not smooth margin ill- defined margins, lobulated or irregular margins, extra-thyroidal extension), and microcalcifications. The concordance of the TIRADS classification in children were similar to that in adults

Conclusions: Ultrasound is high valuable in distinguishing benign and malignant thyroid nodules in children. The ACR-TIRADS 2017 classification can be applied in children.
