Bối cảnh: Phẫu thuật cắt gan trái nội soi (LLLS) được chỉ định cho các khối u gan nằm ở vị trí dễ tiếp cận (mặt cắt bên trái, thùy trái,...), kích thước nhỏ,... Tính khả thi, an toàn còn phụ thuộc vào trình độ phẫu thuật viên, trang thiết bị.
Mục tiêu: Đánh giá kết quả ngắn hạn của LLLS trong điều trị ung thư biểu mô tế bào gan. Đối tượng và phương pháp: nghiên cứu mô tả cắt ngang, gồm 13 bệnh nhân được chẩn đoán mắc ung thư biểu mô tế bào gan ở mặt cắt bên trái của gan, được chỉ định LLLS tại Khoa Phẫu thuật gan mật tụy - Bệnh viện K Trung ương từ tháng 8/2018 đến tháng 4/2019.
Kết quả: Tỷ lệ nam:nữ là 5,49: 1. Độ tuổi trung bình là 58,46 ± 8,67 tuổi (38 - 77 tuổi). Có 30,8% bệnh nhân mắc virus viêm gan B. Có 46,2% bệnh nhân mắc chứng nghiện rượu. 100% bệnh nhân có Child - Pugh A. Tỷ lệ thành công của LLLS là 100%. Không có tai biến trong khi phẫu thuật nào được ghi nhận. Thời gian phẫu thuật trung bình là 163,08 ± 14,94 phút (140 - 180 phút). Không có bệnh nhân nào cần truyền máu trong khi phẫu thuật. Không có biến chứng sau phẫu thuật. Thời gian nằm viện trung bình sau phẫu thuật là 7,54 ± 1,15 ngày (6-10 ngày). Không có tử vong sau phẫu thuật trong vòng 30 ngày.
Kết luận: LLLS để điều trị ung thư biểu mô tế bào gan là một phẫu thuật đơn giản, an toàn, khả thi cao với nguy cơ biến chứng thấp, phục hồi sau phẫu thuật nhanh và thời gian nằm viện ngắn.
