Tóm tắt
Mục tiêu: Mô tả và báo cáo kết quả kỹ thuật Snodgrass cải tiến (KTSC) điều trị lỗ tiểu thấp ở trẻ em.
Đối tượng và phương pháp nghiên cứu: Hồi cứu báo cáo loạt ca bệnh LTT được phẫu thuật theo KTSC bởi cùng 1 phẫu thuật viên tại khoa phẫu thuật nhi, bệnh viện Xanh Pôn từ tháng 7 năm 2016 đến tháng 10 năm 2021. Chúng tôi đã cải tiến kỹ thuật Snodgrass với đường rạch máng niện đạo đến tận đỉnh quy đầu và thêm 1 lớp đệm bọc niệu đạo bằng tổ chức của vật xốp. Các bệnh nhân được kiểm tra nong niệu đạo thường quy sau 2 tuần sau mổ và được theo dõi tái khám thường xuyên.
Kết quả: 153 bệnh nhân (BN) LTT thuộc đối tượng nghiên cứu. Tuổi trung vị là 3 tuổi (từ 1 tuổi đến 13 tuổi). Có 61 BN (39,9%) LTT thể xa; 92 BN (60,1%) LTT thể gần. 9,8% DV cong > 300 sau giải phóng xơ mặt bụng và được làm thẳng bằng kỹ thuật Baskin. Thời gian mổ trung bình là 84 phút (từ 55 - 140 phút); thời gian nằm viện trung bình 9 ngày (từ 7 - 15 ngày). Với thời gian theo dõi sau ra viện từ 2 tháng đến 63 tháng, biến chứng xảy ra ở 20 BN (13,1%), trong đó rò niệu đạo 10 BN (6,5%), tụt lỗ sáo 6 BN (3,9%), cong dương vật 4 BN (2.6%). Không có BN nào bị hẹp miệng sáo hay niệu đạo.
Kết luận: Kỹ thuật Snodgrass với cải tiến của chúng tôi cho kết quả tỷ lệ rò niệu đạo khá thấp và không có BN nào bị hẹp miệng sáo hay niệu đạo sau mổ.
