Background: The Milan System for Reporting Salivary Gland Cytopathology (MSRSGC) is a standardized, risk-based reporting framework that defines diagnostic criteria for salivary gland lesions, unifies cytologic terminology, and assigns an estimated risk of malignancy (ROM) to each diagnostic category, accompanied by evidence-based management recommendations. Despite its recognized clinical utility, the system has not yet been widely adopted in routine cytopathology practice in Vietnam.
Methods: A descriptive cross-sectional study was conducted at the Vietnam National Oncology Hospital (K Hospital) of from January 1, 2025 to December 31, 2025. A total of 687 cases underwent fine-needle aspiration cytology (FNAC), of which 175 cases had histopathological results. The basic characteristics were calculated using appropriate statistical formulas. The Risk of malignancy (ROM) was calculated for each diagnostic category. The aim is to determine the sensitivity, specificity, overall diagnostic accuracy, positive predictive value (PPV), and negative predictive value (NPV) of the method.
Results: The Risk of Malignancy (ROM) for each MSRSGC category was 6,7%, 7,1%, 17,6%, 1,3%, 25%, 90%, and 100%, respectively. For the diagnosis of malignancy, the sensitivity was 88,5%, specificity 99%, accuracy 96,9%, positive predictive value (PPV) 95,8%, and negative predictive value (NPV) 97,1%.
Conclusions: In this study, the MSRSGC demonstrated high feasibility and strong diagnostic performance in the cytological classification of salivary gland lesions. We recommend its routine implementation in clinical practice to facilitate accurate risk stratification and guide appropriate patient management.
