Đặt vấn đề: Nghiên cứu nhằm nhận xét đặc điểm chẩn đoán và kết quả điều trị nhiễm BK virus ở các bệnh nhân ghép thận tại Trung tâm Ghép tạng, Bệnh viện Hữu nghị Việt Đức.
Đối tượng, phương pháp: Các bệnh nhân được ghép thận và theo dõi tại phòng khám ghép tạng thuộc trung tâm ghép tạng từ tháng 1 năm 2015 đến tháng 9 năm 2021.
Kết quả: Tỉ lệ nhiễm BK máu (nồng độ BK trong máu > 250) chiếm tỉ lệ từ 2,0 - 12,7%. Cao nhất ở tháng thứ 3 và tháng thứ 6. Tỉ lệ nhiễm BK niệu chiểm tỉ lệ từ 13,3 - 35,3%. Tỉ lệ nhiễm BK niệu > 107 cao nhất ở tháng thứ 3 (19,4%). Hầu hết các trường hợp đều đáp ứng với giảm liều UCMD đơn thuần chiếm 85/106 BN (80,2%). Có 4 TH (3,77%) không đáp ứng với điều trị và được sinh thiết thận chẩn đoán bệnh thận do BKV, trong đó có 2 bệnh nhân suy thận ghép phải lọc máu chu kỳ. Trong nhóm nhiễm BK máu thể hoạt động sau 6 tháng đã có sự tổn thương thận rõ rệt, biểu hiện qua sự tăng đột biến về chỉ số Creatinin trong máu.
Kết luận: Nhiễm virus BK là một trong những nguyên gây tổn thương thận ghép, đặc biệt tỉ lệ nhiễm cao nhất trong 1 năm đầu sau ghép, Chiến lược điều trị nền tảng đối với bệnh thận do BK virus là giảm liều thuốc ức chế miễn dịch, giảm liều cả 2 loại thuốc ức chế calcineurin (Tacrolimus, Cyclosporin) và thuốc chống chuyển hóa (MMF, MPA) từng bước dựa vào việc theo dõi nồng độ virus BK trong máu.
